Records and clarification

Prepare clearer documentation questions.

When records are confusing, make the questions specific enough for the responsible supervisor to review. This guide does not examine your records, count hours or determine whether past fieldwork will be accepted.

  1. Identify the records relevant to your question

    Locate your existing supervision agreements, tracker, verification documents and prior instructions in their approved storage. Do not upload them here. Note privately which period or discrepancy you need explained.

  2. Separate what you know from what needs review

    Identify the unclear entry, missing explanation or difference between records. Ask which source and applicable requirements should guide the review. Avoid assuming a difference is harmless or automatically disqualifying.

  3. Ask about the authorized next step

    Bring the question to the supervisor responsible for the work and documents. Ask how any correction should be handled and retain the explanation in the appropriate system. Do not use this planner to rewrite signed documents or generate verification.
