Monthly vs. Final Fieldwork Verification Forms: What Actually Has to Match

Somewhere along the way, a lot of BCBA trainees begin thinking of the Final Fieldwork Verification Form as the “real” form and the Monthly Fieldwork Verification Forms as supporting paperwork that probably will not matter once the final totals are signed.


That is not how this works.


Your Final Fieldwork Verification Form is a summary. The Monthly Fieldwork Verification Forms underneath it are what support that summary.


The Final Fieldwork Verification Form is submitted with your certification application. Your Monthly Fieldwork Verification Forms are not submitted with the initial application, but you must keep them because the BACB may request them, including during a fieldwork audit.


A correctly completed Final Form does not reach backward through time and repair missing Monthly Forms, late signatures, incorrect totals, or a BCBA fieldwork tracker that tells a different story.


I have reviewed more than 500 sets of fieldwork records, and the problems are usually smaller than people expect. One month is off by a few hours. Concentrated hours were multiplied before they went onto a form. The tracker says one thing and the Monthly Form says another, but nobody remembers why.


Each record can look perfectly reasonable on its own. The problem shows up when you put everything side by side.


So that is what we are doing here: what each form is supposed to show, what needs to match, and what to do when it does not.


Monthly Fieldwork Verification Form vs. Final Fieldwork Verification Form


The easiest way to understand these forms is to stop treating them as interchangeable.


Monthly Fieldwork Verification Form

Final Fieldwork Verification Form

Documents one monthly supervisory period

Summarizes a completed fieldwork experience

Completed for each month in which fieldwork is accrued

Completed when fieldwork with a supervisor or organization ends

Must be signed by the required monthly deadline

Completed and signed when the fieldwork experience concludes

Retained by the trainee and supervisor

Submitted with the BCBA certification application

Not submitted with the initial application

Reports the hours you are asking the BACB to accept

May be requested during a fieldwork audit

Must be supported by the corresponding signed Monthly Forms


The Monthly Fieldwork Verification Form is commonly shortened to M-FVF.


The Final Fieldwork Verification Form is commonly shortened to F-FVF.


They may sound like two versions of the same document, but they do different jobs.


The Monthly Form verifies what counted during one month. The Final Form summarizes what counted across the completed supervision experience.


To keep this readable, I will call them the Monthly Form and Final Form from here on.


What does a Monthly Fieldwork Verification Form document?


Your Monthly Form verifies the eligible fieldwork hours for one calendar month. It identifies the trainee, supervisor, month, fieldwork type, total hours, supervised hours, supervision percentage, and the other information required by the applicable form.


Both parties sign it. That signature is not simply saying the arithmetic looks right. It confirms that the hours reported on the form are the hours being verified for that month.


Your tracker may show more hours than the Monthly Form for a legitimate reason. Some hours may have been reduced or excluded because a monthly requirement was not met.


But the difference should be traceable.


If the tracker says 120 hours and the signed form says 96, you and your supervisor should be able to explain exactly where those 24 hours went.


When does the Monthly Form have to be signed?


The Monthly Form must be signed by the last day of the calendar month following the month in which the supervision occurred.


If the fieldwork occurred in January, the form must be signed no later than the last day of February.


Not 30 days after the final supervision meeting.


Not whenever the semester ends.


Not when the supervisor finally has time to review six months of records at once.


The deadline is tied to the calendar month. A missing Monthly Form, or one that was not signed by the required deadline, can place that month’s hours at risk.


This is one reason I strongly recommend reviewing the tracker and completing the Monthly Form as soon as the month ends.


Waiting until the final possible day does not make the documentation more accurate. It only gives everyone less time to notice that something is missing or does not match.


Are Monthly Forms submitted with the application?


No.


Monthly Forms are retained by the trainee and supervisor. They are not submitted with the initial BCBA certification application.


That does not make them optional.


The BACB may request your Monthly Forms and supporting records, including during a fieldwork audit. The Monthly Forms are also what support the totals entered on your Final Form.


Do not assume that the BACB will never see them simply because they are not uploaded with the initial application.


Keep a complete copy of every signed Monthly Form.


What does a Final Fieldwork Verification Form document?


The Final Form summarizes the eligible hours completed during a fieldwork experience with a supervisor or coordinated organization. It is completed when that fieldwork experience ends.


That may happen because:


  • You completed all of your fieldwork

  • You changed supervisors

  • You left an employer or organization

  • Your supervision contract ended

  • You changed from one supervision structure to another

  • You paused fieldwork and later began a new supervision experience


The Final Form is submitted with your BCBA certification application.


It is not necessarily one document covering every fieldwork hour you have ever completed.


A trainee may have multiple Final Forms when fieldwork was completed under different supervisors, organizations, supervision structures, or periods. Those Final Forms can be submitted together to demonstrate that the combined experiences satisfy the total-hour requirement.


Each Final Form must be supported by the Monthly Forms connected to that specific fieldwork experience.


What has to match between the Monthly and Final Fieldwork Verification Forms?


The hours reported on the Final Form must be supported by the corresponding signed Monthly Forms.


The Final Form should not report more hours than the applicable Monthly Forms support.


The grand total is not the only thing to compare. I would check:


  1. Names and BACB identification information. Make sure the trainee and supervisor information is accurate and consistent across the forms.

  2. The supervision period. The dates on the Final Form should make sense in relation to the Monthly Forms, the supervision contract, and any supervisor or organization changes.

  3. The fieldwork type. Supervised Fieldwork and Concentrated Supervised Fieldwork are not interchangeable labels. A month cannot be relabeled at the end simply because the concentrated multiplier would be more convenient.

  4. Supervised, independent, and actual hours. The Final Form should be supported by the Monthly Forms. If you completed concentrated fieldwork, use the hours you actually accrued—not the total after applying the 1.33 multiplier.

  5. The supervision structure. Hours completed with an individual supervisor should not be carelessly combined with hours completed through a coordinated multiple-supervisor organization. The forms should reflect how the supervision was actually arranged.


A fieldwork tracker can look completely normal while one of these pieces is wrong.

That is why comparing only the grand total is not enough.


Does the Final Form have to equal every hour in your tracker?


Not necessarily.


Your tracker is your ongoing documentation system. It should preserve the details of what occurred, including dates, times, activities, supervisors, classifications, and the other information needed to support your fieldwork.


The Monthly and Final Forms report the eligible hours being verified.


Those numbers may differ from the raw tracker total for a legitimate reason.

For example:


  • A month exceeded the maximum allowable hours

  • The supervision percentage was insufficient

  • Too few supervisor-trainee contacts occurred

  • A required client observation was missed

  • Group supervision had to be reduced

  • Some activities were not eligible

  • A month was excluded from the verified total

  • The tracker contained all actual hours while the Monthly Form reported adjusted eligible hours


The question is not simply:

Are the numbers identical?

The better question is:

Can every difference be explained by the records and the requirements that applied?

A difference you can clearly trace and explain is very different from a difference that appears because nobody reviewed the records for 18 months.


Common reasons the forms do not match


The discrepancies I find usually come from a handful of places:


  • A missing or late Monthly Form. The tracker may contain detailed entries, but the tracker does not replace a signed Monthly Form.

  • The wrong fieldwork type or the concentrated multiplier. A month may be labeled concentrated even though it did not meet the concentrated requirements, or someone may have multiplied the concentrated hours before entering them on the form.

  • Duplicate months or duplicate time. One month may have been copied twice, or the same period may have been entered more than once and counted twice toward the total. An activity can legitimately be both supervised and unrestricted. The problem is counting the same time twice—not assigning the same hour more than one accurate classification.

  • Separate supervision experiences were combined incorrectly. Hours from unrelated supervisors or organizations may have been placed onto one Final Form even though separate forms were needed.

  • A correction was made in only one place. Someone corrected the tracker but not the Monthly Form, or corrected a Monthly Form without updating the final calculation.

  • Small clerical problems accumulated. Minutes may have been converted inconsistently, formulas may have rounded differently, or fields may have disappeared when a PDF was saved.


None of those problems is especially exotic. That is exactly why they are easy to miss.


After saving a completed form, close it and reopen the saved copy. Make sure every field, selection, date, and signature is still visible before you consider it finished. The BACB specifically recommends reopening saved Final Forms because missing saved data can result in a form not being accepted.


What should you do if the Monthly and Final Forms do not match?


First, do not panic.


Second, do not begin changing signed documents simply to force the totals to agree.


Gather:


  • Your complete BCBA fieldwork tracker

  • Every signed Monthly Form

  • Every completed Final Form

  • Your supervision contracts and amendments

  • Records showing when supervisors or organizations changed

  • Documentation showing whether each month was Supervised or Concentrated Fieldwork

  • Any calculations used to adjust monthly hours


Then organize the Monthly Forms by supervision experience.


For each Final Form, identify exactly which Monthly Forms are supposed to support it.


Add the eligible hours from those Monthly Forms and compare them with the Final Form.


If the totals do not match, work backward month by month until you find where the difference began.


The source may be:


  • One omitted month

  • One duplicated month

  • A calculation error

  • Multiplied concentrated hours

  • A form assigned to the wrong supervisor or organization

  • A tracker total that was never adjusted

  • A correction that appeared in one record but not another

  • A month reported under the wrong fieldwork type

Once you understand the source of the discrepancy, involve the supervisor responsible for that fieldwork period.


Any correction should be supported by the underlying records and completed appropriately with the responsible supervisor. Depending on when an error is found, the correction may require a newly signed form or supported changes initialed by everyone involved.


Do not independently alter a signed verification form merely because a different number would now be more convenient.


A correction should make the records accurately reflect what happened. It should not be used to create a cleaner version of what someone wishes had happened.


What if you had multiple supervisors?


The correct forms depend on how the supervision was structured.


Multiple supervisors at one coordinated organization


When several supervisors provide fieldwork supervision through one coordinated organization, there must be a responsible supervisor who oversees the experience and ensures that the applicable requirements are met.


The responsible supervisor is listed on and signs the Monthly Form.


The Final Form identifies the supervisors who provided supervision through the organization, while the responsible supervisor signs the Final Form.


The supervision contract should identify the participating supervisors, describe their roles, and name the responsible supervisor.


Independent supervisors or separate organizations


If you completed fieldwork under separate supervisors or organizations that were not part of one coordinated supervision structure, you may need separate Monthly and Final Forms.


The monthly requirements may also need to be satisfied independently within each fieldwork experience.


Hours from two unrelated settings do not automatically become one coordinated fieldwork experience just because they happened during the same month.


What changes with the 2027 forms?


Applicants applying under the 2027 BCBA fieldwork requirements must use the corresponding 2027 verification forms.


If you are completing fieldwork during the transition and do not know whether you will apply before or after January 1, 2027, the BACB recommends meeting both sets of requirements and completing both versions of the Monthly and Final Forms with your supervisor.


Completing a particular version of a form does not change what happened during the month. The actual fieldwork must satisfy the requirements represented by that form.


A 2027 form cannot create observation minutes that did not occur.


A 2022 form cannot make 160 monthly hours fit within an earlier 130-hour monthly maximum.


The basic submission distinction remains straightforward:

  • Final Forms are submitted with the certification application.

  • Monthly Forms are retained and are not submitted with the initial application.

  • Monthly Forms and supporting records may be requested by the BACB, including during an audit.

What if a supervisor refuses to sign?


Start with the supervision contract and the underlying records. Try to resolve the issue directly with the supervisor, and keep copies of your tracker, signed Monthly Forms, supervision records, communications, and anything showing that the agreement’s requirements were met.


A dispute involving a Final Form may qualify for the BACB’s contested-fieldwork process. An unsigned Monthly Form is different; that process does not turn it into a signed form.


This is exactly why the supervision contract should explain documentation expectations and the conditions for signing before fieldwork begins.


Finding out at the end that the trainee and supervisor understood the agreement differently is a miserable situation for everyone involved.


Keep every version of your records


Do not discard your fieldwork records after the Final Form is submitted.


Both the trainee and supervisor should retain complete fieldwork documentation for at least seven years.


Keep your own secure copies of:


  • Every signed Monthly Form

  • Every signed Final Form

  • Every supervision contract and amendment

  • Your complete fieldwork tracking records

  • Records supporting required contacts and observations

  • Documentation supporting any corrections or adjustments


Do not keep the only copy:


  • On an employer-owned computer

  • Inside a work email account you may lose

  • In a supervisor’s private files

  • Inside a tracking platform you no longer control

  • On one laptop with no backup


I would organize the records first by supervisor or organization and then by month.

Boring? Absolutely.


Still less painful than trying to locate a supervisor from four years ago because you cannot find one signed form? Also absolutely.


The five-minute review I would do every month


At the end of every month:


  1. Finish your tracker entries.

  2. Confirm that the monthly requirements were met.

  3. Calculate the eligible hours.

  4. Compare the tracker totals with the Monthly Form.

  5. Review the form with your supervisor.

  6. Sign it before the deadline.

  7. Save the completed form.

  8. Close it and reopen it to confirm that every field and signature remains visible.

  9. Store a copy somewhere you personally control.


That process does not guarantee that no question will ever arise.


It does prevent an awful lot of avoidable confusion.


When an independent documentation review may make sense


Not every trainee needs to pay someone to review their fieldwork documentation.


A second review may be worth considering when:


  • Your Monthly and Final Forms do not match

  • You changed supervisors or organizations

  • You completed both Supervised and Concentrated Fieldwork

  • Your records span several years

  • You used more than one tracker

  • Some forms are missing or were corrected

  • You are unsure which requirement set applies

  • You are preparing to submit your certification application

The simplest way to think about the forms


Your tracker records the underlying fieldwork.


Your Monthly Forms verify the eligible hours for each month.


Your Final Form summarizes the completed fieldwork experience.


They do not have to be identical, but they do need to tell the same story. Any difference should be traceable, explainable, and supported by the records.


Compare them while the fieldwork is still happening.


Fixing a discrepancy from last month is inconvenient. Trying to reconstruct it several years later, after the supervisor is gone and the tracker is inaccessible, is an entirely different problem.
